SLC Facility and Event Request Form
Please fill out a Facility and Event Request Form if you are a ministry team leader planning an SLC event, or if you would like to use our facility for personal reasons. *Please submit form 30 to 365 days in advance of the event. *Please allow 7-10 days for review.
*SLC reserves the right to cancel or postpone any event on the calendar with or without expressed cause due to other event priorities or the event's alignment with SLC's vision.
